Perception 1? Are you sure you're not just playing a character that forgot their glasses at home? Jokes aside, sounds like a debuff. Rad poisoning, injuries, or even some quirky gear might be the culprit.

One word: Bug. Fallout 4 isn't exactly glitch-free. If you can't find a legit reason for the drop, might be time to reload an old save.
